WriteAid This form allows students to submit papers for asynchronous, online tutorial feedback from WriteAid.
Challenge Exam The Berkeley College Challenge Exam is optional for Berkeley College students entering their undergraduate (Associates's or Bachelors's) degree programs. Matriculated students can choose to take a Berkeley College Challenge Exam to demonstrate mastery of selected courses that the College requires as part of a degree program.Please note not all courses can be challenged. Students are encouraged to speak to their Academic Advisor for additional information.
Disability Services
Reserve a Proctor Form Students with approved ADA plans can submit this form to the Office of Disability Services to reserve a proctor.
Admissions
International Student Scholarship Application International Admissions Applicants can complete the International Scholarship Application for consideration of an International Award applied toward tuition.
Honors Program In order to apply for the Berkeley College Honors Program, please complete the Honors e-form application and Honors e-Acknowledgment. Once your Honors application is previewed, the three-member Honors Faculty Admissions Committee will review and recommend a final status - Accept, Rejected, or Waitlist usually within 3-6 business days.
Request for HS Transcript Students may request a copy of their high school transcript and provide it to their high school guidance department.
Meningitis As a commuting student after learning about Meningitis, you are waiving the vaccination? All resident students must have a vaccination.
Immunization This form is completed by medical caretaker prior to registration.

SAP Appeal Form The appeal is required for students who are currently not meeting academic standards and for review by the Satisfactory Academic Progress (SAP) Appeal Committee.
Grade Appeal form

The Grade Appeal e-Form provides students the opportunity to request a review of the grade they received through, if needed, potentially three levels of appeal - first to their professor, second to the department chair and a third and final appeal to the School Dean.

Incomplete Grade Contract

A student requesting a grade of Incomplete must submit this eform to the professor who will then respond with a decision. If the request is approved by the professor, the student must acknowledge all contract conditions and sign the returned form.

Withdrawal This form is to withdraw from individual courses, withdraw from college during the term or to be withdrawn from the college at the end of the term, once classes end.
Course Reinstatement Form This form is to be filled out when seeking reinstatement from a withdrawn course in the current term. Reinstatement processed pending approval from the Professor.
Leave of Absence Students seeking a Leave of Absence must fill out this form for review and an approved Leave of Absence (LOA).
MUCW Readmissions Application This form is to be filled out when a student on a MUCW withdrawal is ready to readmit to Berkeley College.

Employment Assistance Application (EAA) Form to be submitted by current students to request Career Services assistance or update employment information.
Student Records (FERPA)
FERPA Authorization to Release Student Records Student authorizes the college to release designated types of education records to designated people.
FERPA Opt-Out of Routine Sharing of Student Directory Info Student tells the College NOT to release “directory info” (defined in our FERPA Policy) outside the college.

PLUS Loan Cancellation Request A parent who would like to cancel a Federal Parent Loan Disbursement must use this form.

Authorization To Reduce Loans-Younger than 18 This form allows the College to reduce your federal loans should your other aid increase.
Authorization To Reduce Loans-18 years of age and older This form allows the College to reduce your federal loans should your other aid increase.

Professional Judgment Financial Aid can adjust information provided on the FAFSA, called Professional Judgment (PJ), to better reflect the financial need of students and families. The Professional Judgment page will provide an understanding of each category of professional judgement, supporting documentation needed, and how students can initiate a PJ.
Student Accounts
Clearance Request This eform can be used by students to request registration clearance with the Student Accounts Department for the upcoming semester. This eform will show total account charges and detailed financial aid for the semester as well as give various options for obtaining clearance including payment in full, payment plan, 3rd Party/Employer Reimbursement, or Military/Veteran Benefits.
Request Statement of Account This eform is used by students who would like to receive a statement of their account that shows tuition, other charges, financial aid, etc.
Transfer Fund Request This eform is used by students who would like up to $500.00 of their financial aid credit transferred to their Berkeley ID’s so that they can buy books and supplies from the bookstore.
Third Party Billing Agreement This eform is used by students who participate in a Tuition Reimbursement program where their employer covers all or a portion of their tuition and fees and/or balance. It is a clearance requirement each semester.
Veterans Billing Agreement This eform is used by students who participate in Veteran benefits program where the Department of Veteran Affairs and/or Tuition Assistance program covers all or a portion of their tuition and fees. It is a clearance requirement each semester.
